Clinical Xpert CareFocus is a web and mobile-based patient profiling system.
CareFocus gives hospitals, RHIOs and other healthcare organizations a powerful
tool to identify at-risk patient populations by automatically identifying and
managing hospital or clinician defined patient populations.
CareFocus lets hospitals run custom queries against the hospital patient data
in order to create target lists of high-risk patients. Queries usefully customizable
profiles, combining factors that include lab values, risk factors for different
conditions, age, gender, prescribed drugs and more.
Typical patient lists include diabetic patients, patients with congestive heart
failure, 23-hour observation beds, potential asthmatics, and infectious disease
patients. Lists are distributed to clinicians´ desktops and mobile devices via Clinical
Xpert Navigator and Clinical Xpert Mobile Navigator.
Clinical consulting services (training and support for building and managing
clinical profiles) are available through Thomson Healthcare´s Xpert Services
team.
