Clinical Xpert Handoff eliminates manually written handoff lists by electronically
generating a single, centralized list for each care team. The application generates items
for tracking diagnosis, code status and allergies, along with a problem list integrated
with relative patient information.
Each care-team member subscribes to a team list and then accesses the handoff lists
from which they can track and edit the Dx, code status, allergies, and current to–do
list for all of the team´s patients via both the web and mobile devices.
By eliminating manual data duplication and simplifying the handoff process clinicians can
save between 1.5 and 3 hours per week. Handoff increases communication and collaboration
between care team members through a single, centralized handoff list–reducing the potential
for medication errors. Handoff assists with ensuring compliance with JCAHO and NCQA guidelines
for patient safety and care.
